Shop for new and gently used clothing, jewelry, antiques, collectibles, furniture, appliances, housewares, books and more at our Community Hospice Thrift Shop and Thrift by the Pound locations in Jacksonville and Orange Park.
Community Hospice & Palliative Care has approximately 1,300 people in our hospice care per day.
In addition, our Community PedsCare (peeds care) provides palliative and hospice care for almost 300 pediatric patients. Our PedsCare program is a minimally funded program, so much of the Thrift Shop proceeds go to maintaining this program. In addition, we provide Caregiver Services, Advance Care Planning, Grief and Bereavement services regardless of whether they were in our program or not. We also have a Veteran-centric program and our unfunded services include a Certified Music Therapist and a Certified Massage Therapist. We employee more than 900 staff and have more than 1,000 volunteers to provide care for our patients and their families.

Since 1979, Community Hospice & Palliative Care has been the Compassionate Guide for advanced illness management, helping people live better with a chronic condition.

In addition to providing hospice care, Community Hospice & Palliative Care offers several programs that help improve quality of life, such as Community PedsCare®, a nationally recognized pediatric palliative and hospice program for children with life-threatening and life-limiting conditions; Community Hospice & Palliative Care Veterans Partnership, a program providing specialized care and support to address the unique needs of Veterans; Community Palliative Care, a consultative service for people with serious and complex illnesses that helps them make important health care decisions; and Community Care Choices, a CMS research model to help people with advanced illness find greater access to comfort care and support.

The Community Hospice & Palliative Care Neviaser Educational Institute provides education and other resources for health care professionals and the community about dealing with serious illnesses and their impact.
